TERMS AND CONDITIONS WITH AGREEMENT<\/strong><\/p>

This Agreement is entered into as of the ___ day of __________, 2024, by and among the Team Members (collectively referred to as the “Team”) listed below to outline their collective responsibilities, commitments, and entitlements for promoting and selling COLONICARE products as part of an Affiliate Group Account.<\/p>

TEAM SIZE LIMIT<\/strong><\/p>

  • Each Affiliate Group Account can consist of a maximum of 10 members.<\/p><\/li><\/ul>

    TEAM MEMBERS:<\/strong><\/p>

      AFFILIATE GROUP ACCOUNT PARTICIPATION<\/strong><\/p>

      Single Affiliate Link<\/strong><\/p>

      \u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 The Team will operate under a single Affiliate Group Account, using a shared affiliate link to track all sales and commissions.<\/p>

      \u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 The members shall appoint and agree to register the account under the assigned Team Leader’s name.<\/p>

      Commission Structure<\/strong>:<\/p>

      \u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 The Team will collectively earn a 25% commission<\/strong> on all direct sales through their affiliate link.<\/p>

      \u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 An additional 5% multi-level commissions<\/strong> will apply to sales generated by 2nd, 3rd, and 4th-level invites as part of the affiliate program.<\/p>

      Group Responsibility<\/strong><\/p>

      \u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Team members share responsibility for managing the affiliate link. The assigned treasurer within the Team will distribute all commissions earned according to their internal Agreement.<\/p>

      TEAM LEADERSHIP AND ROLES<\/strong><\/p>

      \u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Team Leader<\/strong>: The Team will assign a leader to oversee activities, coordinate efforts, and ensure the responsibilities of each member are in accordance. The Leader’s role includes being the primary affiliate link holder for tracking sales.<\/p>

      \u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Treasurer<\/strong>: The Team will designate a Treasurer to receive and manage all commission distributions from the affiliate program.<\/p>

      \u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Leadership Adjustment<\/strong>: The Team may vote to change the Leader if they feel the current Leader is not performing effectively.<\/p>

      TEAM COLLABORATION AND CONTRIBUTION<\/strong><\/p>

      \u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Effort Requirement<\/strong>: The primary tasks include finding clients or patients interested in purchasing COLONICARE products or packages and becoming resellers. All members shall actively contribute in performing these tasks.<\/p>

      \u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Product Promotion<\/strong>: Team members educate potential clients on product benefits, usage, and reseller earnings potential.<\/p>

      \u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Non-Participating Members<\/strong>: Members who fail to participate to put in effort may forfeit their commission and may be terminated from the Team by a majority vote.<\/p>

      SALES TARGET AND MONTHLY ALLOWANCE<\/strong><\/p>

      \u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 The Company will provide a monthly marketing allowance of 5,000 PHP if the Team collectively reaches a target of 133 packs<\/strong> sold (including reseller packages containing four packs).<\/p>

      \u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Failure to meet the 133-pack target will result in the cancellation of the allowance for the following month.<\/p>

      PROFIT DISTRIBUTION<\/strong><\/p>

      \u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 All profits will be distributed equally among active Team members every week.<\/p>

      \u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 The Treasurer will manage the distribution of commissions earned from direct and multi-level sales.<\/p>

      DOKFAT WELLNESS CARD MEMBERSHIP AND BENEFITS<\/strong><\/p>

      DokFat Wellness Card<\/strong><\/p>

      \u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 The Team will receive a DokFat Wellness Card under the Leader’s name, which all active Team members may use.<\/p>

      Exclusive Discounts<\/strong>:<\/p>

      \u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Team members are entitled to an exclusive 10% Discount<\/strong> on therapy treatments at DokFat Naturopathic Healthcare Services.<\/p>

      \u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Team members can enjoy a 20% Discount<\/strong> on Pure Organic Supplements at DokFat Pharmaceuticals.<\/p>

      \u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 To receive these discounts, Team members must present the DokFat Wellness Card during each visit<\/p>

      \u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Membership benefits provided under the DokFat Wellness Card are exclusive to the assigned Team and cannot be transferred to others outside the Team.<\/p>

      MARKETING MATERIALS<\/strong><\/p>

      \u00b7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 The Company will provide the Team with marketing materials to support product promotion.<\/p>

      TERMINATION AND ADJUSTMENTS<\/strong><\/p>

      The Team acknowledges that any member who does not actively participate may lose their eligibility for commission payments and may be removed from the Team by consensus. This Agreement may be reviewed and updated with mutual consent to ensure Team goals are achieved efficiently.<\/p>
